FIELDHOME has taken its place as a regional leader in sub-acute rehabilitation/skilled care and, thanks to some significant service enhancements, is now without question, the facility of choice. In late 2006, FIELDHOME began a new era in care through the welcome addition of full-time Doctor of Rehabilitation, Arefin Siddique, M.D., and the grand-opening of a brand new 1,500 sq. ft. Rehabilitation Treatment Center with separate Occupational Therapy Suite. FIELDHOME offers a 50-bed newly renovated and decorated Rehab Unit with a large number of private rooms. Short-Term Rehab Services offered include: Post-Surgical Orthopedic Care (specializing in Joint Replacement Rehab); Sub-Acute Care; Cardio/Pulmonary Rehab; Wound Care; Medically Complex Care. Skilled Nursing

FIELDHOME also offers both long-term and short-term skilled nursing services. Long-term care residents benefit from well over a century of experience in elder-care services and a fully integrated and interdisciplinary team approach to care. FIELDHOME offers a high nurse to resident ratio and takes pride in it’s staff who are professionally trained to manage special needs such as wound care, infusion therapy, pain management and palliative/comfort care. Short-term skilled nursing services are also available to our rehab patients and can include IV therapy, wound care, medically complex care and more.

Respite and Hospice Programs

FIELDHOME also offers both a Respite and Hospice Program to provide for alternate needs. Our popular Respite Program provides relief for caregivers during vacations, business needs or simply, for a much deserved period of rest and relaxation. Our Respite guests receive the highest level of care and attention enjoyed by all of our patients and residents during their stay. The Hospice Care program is offered to residents who are in need of palliative/comfort care. This program is designed to accommodate the complex needs of end of life patients.
